Registration

Pre-registration is NOT required to attend Evan's Ride For A Cure.  However, by registering early you allow us to properly prepare for the number of t-shirts, meals needed, lunch seating and route maps for the ride.

 Evan's Ride For A Cure. You will directed to the HAA website where you can pay on-line or print out the form and submit via mail with your check.

Pre-registration cut-off date is: June 22, 2009

*If you cannot pre-register the necessary forms will be made available prior to the ride.

Here is what you need to fill out:

1.Registration Form

  2.Rider Waiver/Release Form

Both need to be filled out COMPLETELY with checks made payable to:

Histocytosis Association of America or 'HAA'
